Step One: Complete your 5-year update application and bring it to the church office.

Complete our 5 year update application by downloading and printing or picking up a copy in the church office. When it's complete, bring it to the church office or give it to your ministry leader along with a copy of your photo ID. You will need:

• A copy of your driver's license or other photo ID
• Your social security number

Step Three: Take the quiz

Take our short quiz to review the video content. Once you submit your quiz and turn in your application, we can begin updating your file. Your file is good for five years.
